Medical Spa Treatments for Skin of Color: What You Need to Know

Not all medical spa treatments are equally safe for all skin tones. Here is what clients with darker skin should know before booking any aesthetic procedure.

Clients with darker skin tones — typically Fitzpatrick skin types IV through VI — require specific consideration when evaluating medical spa treatments. The same treatments that are safe and effective on lighter skin can cause adverse reactions including post-inflammatory hyperpigmentation, burns, or scarring on darker skin if not performed with appropriate knowledge, settings, and technique. Being an informed client protects you from preventable complications.

The Key Concern: Post-Inflammatory Hyperpigmentation

The primary risk for clients with darker skin tones undergoing certain medical spa treatments is post-inflammatory hyperpigmentation, or PIH. Darker skin contains more active melanocytes that are more easily triggered to produce excess pigment in response to inflammation or thermal injury. This means that a treatment causing a small amount of inflammation — which would heal without a trace in a fair-skinned client — can leave a dark mark on a client with a darker complexion that may take months to resolve.

Understanding this risk and choosing providers who understand how to minimize it is the most important protective step a client with skin of color can take.

Laser and Light Treatments

Traditional laser hair removal and many laser resurfacing systems were developed primarily for lighter skin types and can cause thermal injury and significant hyperpigmentation on darker skin if used with incorrect settings or incorrect technology choices.

However, darker skin tones are not excluded from laser treatments when the right technology is used. Nd:YAG lasers at 1064nm wavelength are considered safe and effective for laser hair removal on darker skin because the longer wavelength is less absorbed by melanin in the skin surface and more selectively absorbed by the deeper follicle. A provider who offers laser treatments for darker skin should be able to specify exactly which device and which settings they use and why those choices are appropriate for your skin tone.

For skin resurfacing and rejuvenation, non-ablative and fractional technologies at appropriate settings can be used on darker skin types, but require specific expertise and conservative treatment parameters.

Chemical Peels

Chemical peels can be used effectively on darker skin when the appropriate peel type and concentration are selected. Superficial peels using glycolic, lactic, or mandelic acid are generally well-tolerated on darker skin types. Medium-depth peels require more caution and should only be performed by providers with specific experience in treating darker skin.

Trichloroacetic acid peels and phenol peels require significant expertise and conservative protocols on skin of color to prevent hyperpigmentation and scarring.

Injectable Treatments

Botox and dermal fillers are generally safe for all skin tones since they do not involve heat or UV energy that could trigger hyperpigmentation. The same considerations around provider skill and product quality apply regardless of skin tone.

Finding the Right Provider

The most important protective step for clients with skin of color seeking medical spa treatments is finding a provider who has demonstrable experience treating clients with similar skin tones and who can show you examples of their work on patients of color.

Ask specifically how frequently they treat clients with skin tones similar to yours, what specific protocols they follow to minimize the risk of hyperpigmentation, and what they do if post-inflammatory hyperpigmentation develops. A provider who answers these questions specifically and confidently, who has a portfolio that includes patients of color, and who takes a conservative, evidence-based approach to treatment selection is the right choice for your care.

Advocating for Yourself

Clients with skin of color have historically been underserved by the aesthetic medicine industry, partly because less research has been conducted in diverse populations and partly because some practitioners have limited experience with darker skin types. As an informed client, you have every right to ask detailed questions about your provider's experience with skin of color, to see specific examples of their work on patients with similar complexions, and to decline treatment if you are not satisfied with their answers. Your skin health is worth the additional scrutiny, and finding the right provider for your skin type is worth whatever search it requires.

The best outcomes in medical aesthetics consistently come from a combination of choosing the right provider, having realistic expectations, following pre and post-treatment instructions diligently, and committing to the consistency of care that allows treatments to produce their full cumulative benefit. Every investment you make in finding the right practice and building a trusted professional relationship pays dividends across every treatment that follows. Working with a provider who understands your specific goals, assesses your situation thoroughly, and communicates honestly about what is achievable creates the foundation for outcomes that consistently meet or exceed your expectations over the long term of your aesthetic care relationship.
